With custom formulations, you can create tailored templates for IVs, IM injections, herbal blends, and other treatment items. These formulations can be integrated directly into your charting workflow allowing you to document treatment details, track inventory usage, and attach the formulation as a provided service within the chart note.

Inventory
The first step to building your formulations in the system is to add the ingredients used to your inventory. Manage your inventory following the steps listed here.
If the item will be part of a formulation be sure to check the box for which type of formulation.
Please note that all of your IV ingredients need to be input in your inventory per milliliter
- This means your price and cost need to be calculated per mL, as well as any inventory controls.
- For the Size box just type in 1 for 1 ml and report your on hand amount in mls so that your custom formulation set up can properly countdown your current supply when you click on the blue "Save And Update Inventory" button
Build Your Templates
In settings > clinical templates > IV, IM, & Herbal Formulations you can pre-build your formulation templates for quick access and streamlined charting.
To create a template select “Add New Custom Formulation” and choose the template type. Anything you add to the template will be available when accessed through the chart note or the actions menu.
When building out your template be sure to add a name at the top and at the bottom you can pull in your inventory ingredients and define a fixed price if applicable.
Use your Formulations
From The Actions Menu
Choose “Custom Formulations”
Select “Add New Custom Formulation” and choose the formulation type.
To use the template scroll to the bottom and from the templates drop down select the template name. Complete any additional fields you would like and you can either “save” to complete the documentation, or “save and update inventory” which will also deduct the items used from your inventory.
If you also need to charge the patient for this formulation you can do so by selecting the “formulations” button within the superbill
Then select the 3 blue action dots to “Add to Services” or “Add to Procedures”
From the Chart Note
Choose the “More Commands” drop down and “Formulations”
Select “Add New Custom Formulation” and choose the formulation type.
To use the template scroll to the bottom and from the templates drop down select the template name. Complete any additional fields you would like and you can either “save” to complete the documentation, or “save and update inventory” which will also deduct the items used from your inventory.
After you have completed your formulation use the 3 blue action dots to document the formulation in your assessment or plan, and add it to your services provided in the assessment section to document the charge.
Osmolarity
If you add your osmolarity to your ingredients, once added to the templates you will see the system auto calculate the osmolarity for that formulation.
To calculate osmolarity, the program:
- Multiplies the number of mL of a substance by the osmolarity (mOsm/mL) that you have set in your custom formulation from your inventory
- Then adds all of those figures together
- Divides them by the total volume of the mixture
- Then multiplies by 1000 to bring the mOsm/L.
