Knowing if a Chart Note is Locked
You can tell if a chart note is locked by looking at the little lock icon on the Check-out page or Charting History
Unlocking a Chart note
To unlock a chart, go to Shortcuts > Admin > Unlock a chart
Search the patient's name and the date associated with the chart
Click Refresh to find the chart you want to unlock > then, add a reason for unlocking the chart > click the Unlock button
If you are trying to access or unlock a chart and getting the following error message:
"Only Practitioner who created the consult has access to this page. Please request the OptiMantra Administrator in case you are not supposed to see this message or contact support@optimantra.com
You will need to adjust your Charting Access Controls under the Settings > User Management > Manage > Practitioner Sections and "Others can edit my charts" for the provider whose charts you are trying to access.
Adding an Addendum
If you need to add an additional entry to a signed and locked chart note to include information that was not available or omitted at the time of the original documentation you can use an addendum to clarify, update, or correct the existing note without altering the original content, ensuring the medical record remains accurate.
To add an addendum to a chart go to the patient's actions menu > charting history. You can then select the closed "lock" icon.
This will trigger a pop-up addendum screen where you can add an addendum for a locked consult.
