When you build a custom questionnaire in OptiMantra EHR that includes subtotal calculations, you can trend the data over time (answers must be numeric). This is especially useful for tracking how a patient’s responses or scores change across visits. For more information on building custom questionnaires, click here (Configure Forms).

Form Setup
If your questionnaire includes questions with data you want to trend, be sure to set Allow Resume Later to Yes, and make sure the answers to the questions are numeric only. Trending only works when the same form is updated by the patient over time. If the patient completes multiple separate versions of the same form, the data cannot be trended.
When accessing the Patient Portal, patients must select Resume Questionnaire to continue updating the same form and ensure their data is trended correctly.
After the patient has filled out the custom questionnaire, you are now able to trend it!
How to Trend Data from a Custom Questionnaire
- Go to the patient’s Actions menu
- Select Questionnaire
- Click the three dots next to the questionnaire
- Choose View Trend

You can view trending data by individual question or by the questionnaire’s overall score. Use the drop-down menu in the top-left corner to switch between views.
If you’d like to print or download the summary, click the three-line menu in the top-right corner of the summary screen.
