There are several ways to review patient-submitted forms in OptiMantra, depending on your workflow and preferences. This article outlines the available options so you can choose the method that best fits your practice.
Notifications
Within the white toolbar at the top of your OptiMantra screen, you'll find the Notifications icon. This section displays alerts for items that may require your attention. To view forms submitted by patients, make sure to check the box for "Show notifications for all users." This ensures that you're able to see forms submitted to the system.
If you would like an email notification when your patients have completed questionnaires or intakes, you can add your notification email address destination in settings > business > reminder email.
Shortcuts
In Shortcuts > Review Patient Forms you can see all forms submitted by patients. Within this list you can sort by provider, patient, date of submission, form type, and form name. You can also perform actions to view the form, fax it, or assign it to a different patient.
Actions Menu
From the chosen patients actions menu, if you would like to view all forms completed by the patient select “submitted intakes.” This will list all consents, questionnaires, and intakes completed by the patient. If you would like to complete a new form or look at a specific form type you can also view the individual categories for Intakes, Consents, and Custom Questionnaires.
Charting
You can access Questionnaires in the top grey toolbar of the chart note. This may be flagged with a red number if your patient completed a questionnaire prior to the visit.
When you select the Questionnaire button you will have several options:
- Fill a new questionnaire
- Edit or view a completed questionnaire
- Trend individual questions and the summary for scored forms
- Drop the responses into your S,O,A,P or G (basic note) sections
Your chart note is also dual purpose and some subjective sections can be used as an intake form. If you have chosen to use the intake form, from the grey toolbar use the “intake” button to import sections completed by the patient into the applicable chart note sections.
You will have the option to import and apply to the appropriate sections or import, apply to the appropriate sections, and expand the sections so you can review the patient's responses.
