Using The AI Scribe (Beta)

Overview
The AI Scribe is an ambient, AI-powered note-taking feature currently in beta. It listens during a patient visit, or transcribes a previously recorded audio file, and drafts a chart note with Subjective, Objective, Assessment, and Plan (SOAP) sections.
This article explains how to start a scribing session, review the draft note, and populate it into the patient's chart.
Note: This feature is in beta. During beta, feedback on transcription quality is required each time you populate a note. See the Beta Feedback section below.
Before You Start
The AI Scribe control appears at the top of the chart note (initial or follow-up) once it is enabled for your account.
- If you do not see it, contact us to have AI Scribe turned on for your account.
Starting A Scribing Session
- Open a new or existing chart note (initial or follow-up note types are currently supported).
- Locate the AI Scribe control at the top of the note.
- Click Start Scribing before beginning the patient interaction.
- Leave the recording running in the room for the duration of the visit. AI Scribe captures both the practitioner's and the patient's speech and sorts it into the appropriate note sections automatically.
Uploading A Pre-Recorded Audio File
If you recorded the visit outside the software, for example on your phone or through an external telehealth platform, you can upload the audio afterward instead of recording live.
- From the AI Scribe panel, select Upload Audio.
- Click Browse and select the audio file.
- Upload the file and wait for the note to be drafted.
Note: Built-in telehealth visits cannot currently be recorded directly by AI Scribe. If you are on a telehealth visit, you can record an audio file (for example, on your phone, or via a sound recorder program on your computer) and upload it afterward.
Reviewing The Drafted Note
Once the note has finished drafting, click Preview to review the AI-generated content before it is added to the chart.
- Subjective: what the patient reported.
- Objective: findings noted during the encounter.
- Assessment: draft diagnosis, with ICD-10 codes placed in the proper form fields.
- Plan: the plan agreed upon during the visit, including medications.
From this preview, you can:
- Include or exclude any section from the final chart note.
- View the full transcript of the visit.
Beta Feedback (Required)
Because AI Scribe is in beta, hovering over Populate Note will prompt you to provide transcription feedback before the note can be populated.
- Review the drafted sections and any flagged items (for example, tasks that were really patient instructions rather than tasks for clinic staff).
- Enter feedback describing anything that needs correcting. For example, note that certain tasks belonged to the patient, not staff.
- Submit. You will see a confirmation message thanking you for your feedback.
Note: This feedback loop helps us to improve the accuracy of the model, so please flag anything that looks off, not just major errors.
Populating The Note
- Click Populate Note.
- Confirm the Objective, Assessment, and Plan sections have been added to the chart.
- Check the Assessment section. ICD codes should appear in the correct form fields.
- Check the Plan section. Medications should appear as listed.
Sending Prescriptions To MDToolbox
Note: If you plan to send prescriptions to MDToolbox, uncheck the medication entries when populating the note. Otherwise, the medications will be copied twice to the patient's "All Medications & Supplements" panel.
Saving The Note
Once everything looks correct, click Save, Save & Close or one of the other chart completion options. The note is complete.
Frequently Asked Questions
Do I need to do anything special to turn on AI Scribe?
No. Once it is enabled for your account, the Start Scribing control appears automatically at the top of chart notes. If you do not see it, contact us to have it turned on.
Can I use AI Scribe during a telehealth visit?
Not directly through our built-in telehealth visits, since that is not currently supported. You can record a separate audio file and upload it to AI Scribe afterward. We plan to directly integrate this feature into our built-in telehealth platform in the future.
What if AI Scribe picks up a patient instruction as a task for staff?
Uncheck it in the preview before populating the note, and note it in your beta feedback so the model can improve.
Will medications get added twice if I use MDToolbox?
They can, if you do not uncheck the medication entries before populating the note when you are sending the prescription through Rx Toolbox.
Is feedback really required every time?
Yes. During the beta period, transcription feedback is required before you can populate a note. This helps us rapidly improve the model before full release. However, once beta concludes, feedback will become optional.
Can I still see the raw transcript?
Yes. The transcript is viewable from the preview screen alongside the drafted note sections. It also remains viewable if you edit the chart later.
